First Appointment

What should I bring with me to my first appointment?

If you have had any prior X-rays or MRIs, bring those with you. Other than that, please wear athletic clothing and if you walk, jog, or run, you'll want to bring your athletic shoes as well.

Should I fill out any forms prior to attending my first appointment?

YES...Please complete the online New Patient Initial Exam Form at least 48 hrs before your first visit. This saves you from filling out paperwork in the lobby and gives you more time to spend with the Doctor!

What should I wear to my appointments?

We recommend that patients wear athletic clothing to all of their appointments. Athletic clothing will allow unrestricted movement while maintaining your modesty. The typical gym or work-out outfit is best: t-shirt, shorts, capris, sweats, yoga pants, etc. For your comfort, we do recommend bike shorts or spandex be worn beneath shorts and request that no "short" shorts be worn.

Do I need to get an MRI or an X-Ray before coming to the clinic?

No. You do not need an MRI or an X-Ray before coming to see us. If an MRI or X-Ray is required, our Doctors will be able to order one for you.If you happen to have had an MRI or X-Rays from a previous Doctor, please bring those reports with you.

Do I need to get a referral from another Doctor before I can come into your clinic?

No, a referral from another Doctor is not required. You are welcome to schedule an appointment with one of our Doctors any time you feel it is necessary.

Can I send my MRI or Image reports via fax or email?

Yes, you are welcome to fax or email your Image Reports to our office. Be sure to include as much information as possible, including your name, the date and time of your Initial Exam, and the reason for your visit. Our fax number is 713-513-5048

What is the typical wait time in the clinic?

We realize your time is very important. We do not double book appointments and do our absolute best to see you at your scheduled appointment time.

Occasionally we may run a few minutes behind, however this is rare. We do appreciate you being on time so we can give you the full appointment time you have paid for. Being punctual to your appointment helps us stay on schedule.

Please note, if you are more than 15 minutes late to an appointment, we may request you reschedule.

Insurance, Cost & Payment

Do you accept insurance?

We are a fee for service clinic and are treated as an out of network provider by insurance companies. However, all of our notes and billing procedures are done specifically so you can submit your claim and receive an optimal return. We provide you with all of the necessary information to file a claim with your insurance company.

How much does treatment cost?

Each patient case is unique and requires a different number of appointments for resolution. For further information on our fees, please call us at (713) 629-9200 or visit your providers website.

How do I pay for my treatment?

We accept check and all major credit cards. We are a fee for service clinic, so we do expect payment upon receipt of services. We do not send bills or invoices.

Cancellation Policy

What is your late policy?

Our Doctors and Therapists run on a tight schedule and do our best to see our patients at their scheduled time. We do ask that all patients to be on time to their appointments. If you need to change, please arrive at least 5-10 minutes early to give yourself time to do so. If you are more than 15 minutes late, we may ask you to reschedule your appointment.

What is your cancellation policy?

We require 24 hours notice for all cancelled appointments. A $75 fee will be assessed for no-shows.
